Forms and Emergency Card
Dear Parents/Guardians,
The athletics packet for the 2026-2027 school year has been sent home with your student athlete and also posted across multiple district platforms. The athletics packet includes all Rank One and UIL paperwork, emergency cards, and a blank physical form. Rank One paperwork cannot be completed online this year and a paper copy needs to be turned in before the start of next school year. If your student athlete is participating in summer workouts, it should be turned in beforehand or they will not be allowed to participate.
All student athletes 7th-12th grade will need a new sports physical. We are requiring yearly physicals at London. The physical MUST be completed on the UIL approved physical form. Any physicals not completed on the UIL form will not be accepted.
London athletics will be hosting free physicals on June 1st from 4-7 PM in the high school/new gym.
