In May, we are pushing hard to have as many student athletes as possible complete and turn in all paperwork in order to participate in athletics the 2019-20 school year. All necessary forms found below may be printed, filled out and turned in to your child's coach or the Athletic Trainer, Tom Braun. Mr Braun will eventually be the person to review all paperwork and look for errors, missing forms, missed signatures, etc. All physicals and associated athletic paperwork are kept on file in his office.
Every child must fill out a new paperwork packet each year whether a new physical is required or not. Incoming 7th, 9th and 11th graders must see a Dr. of your choice and turn in a properly completed physical that must be completed after May 1st, signed by the Dr. and with that doctor's name, address and the date of the exam legibly printed on the proper UIL physical form. Any other form turned in will be accepted as long as it states that the athlete has no restrictions for athletics. If that statement cannot be found somewhere on the physical form, it cannot be accepted as a valid physical.
There is always some confusion about the Pre participation Physical Evaluation-Medical History form that is attached to the physical examination form. Each student athlete and parents must fill out and sign this form regardless of whether or not a they are required to have a physical.
